Non-Disclosure Agreement (NDA)

All members and affiliates of the PHACS network are required to sign a Non-Disclosure Agreement (NDA).

Please note the definitions of PHACS network members below:

• Site Staff Member*: Site Principal Investigators (PIs) and staff who directly interface with human subjects participating in research.  
     *Site staff who directly interface with human subjects and are listed on their site’s Delegation of Authority and Site Signature log do not need to sign the member/affiliate NDA because they will have already signed the PHACS NDA for Site Research Personnel at their site.
• Member: Individuals who contribute to and fully participate in PHACS activities, but are not site staff (e.g., Other investigators, Harvard program staff and analysts, Frontier Science & Technology Research Foundation (FSTRF) project staff, Westat project staff, Community Advisory Board (CAB), Peers United Group (PUG), Community Task Force Members).
• Affiliate Member: Individuals who have limited participation in PHACS activities.  An affiliate is recommended by a Member (Scientific Leadership Committee (SLC) member or Working Group (WG)/Task Force (TF) Chair) to join the network. 
• Advisory Member: National Institutes of Health (NIH) representatives, External Advisory Board (EAB) members.
